We Bring the Care to You

There are times when a patient may not be able to make it to the office to see their primary care doctor. Such circumstances shouldn’t impede access to quality health care. That is why we launched a program to bring our outstanding providers directly to patients outside of the traditional office setting.

Our Home Visit team provides Enhanced Annual Visits (EAVs) in the home for patients identified as high risk and/or homebound. This extension of care from primary care providers includes:

Ordering durable medical equipment
Completing forms for aide service in home
Hospital follow-ups
Sick visits
Established patient follow-ups
Vatica annual well visits
Flu shots
